Lumane Health
Lumane Health is a care transitions platform built for skilled nursing facilities (SNFs), designed to help patients move safely and quickly into their next setting of care, and to close the coordination gaps that cause preventable hospital readmissions and reduce the need for duplicative, resource-intensive diagnostic imaging scans and testing.
Discharge planning at a SNF is a high-stakes coordination problem: staff need to hand off accurate patient information to the next provider (eg. a primary care physician, specialist, or emergency department) while also identifying a placement that fits both the patient's clinical needs and their insurance coverage. When that handoff is slow or the placement is a poor fit, patients are more likely to bounce back to the hospital, which is costly for the facility and unsafe for the patient. Lumane gives discharge planners a platform to coordinate that handoff and match patients to the right next care setting based on clinical needs and payer coverage, so patients leave with a clear, coordinated plan instead of a gap in their care.
